Forsyth County Department of Public Health-Minority Diabetes Prevention Program

The Forsyth County Department of Public Health is guided by the principles of "Integrity, Awareness, Accountability, Respect, and Excellence" (WeCare). These values shape the work we do to prevent disease, promote a healthy community, and improve residents' overall well-being through regulation, education, and partnerships.

MDPP is a FREE diabetes prevention program that serves 10 counties (Forsyth, Davidson, Yadkin, Surry, Stokes, Wilkes, Davie, Watauga, Alleghany and Ashe). The program helps those who are prediabetic to manage or eliminate their prediabetes through healthy eating habits, physical activity and stress management. MDPP is a 12-month program in which participants attend classes, facilitated by a lifestyle coach, to learn and share how the risk of Type2 diabetes can be decreased/eliminated. During these classes, some of the elements learned are... *Nutrition *How to eat healthy/reduce portions while eating out/vacation *Various types of beneficial physical activity *Ways to manage stress to promote good health *Manage stress triggers *Track activity and food *Read nutrition labels *Set/recognize health goals With the atmosphere being that of a “support group,” participants are encouraged to share their experiences and health journey in the safe space created for them. Participants are rewarded with many incentives that reinforce the elements learned in class. Incentives include kitchen items and appliances, exercise equipment and items to manage and track your activity/intake (to name a few). Participants are celebrated at the end of the year with a graduation ceremony. Enrollment requirements include... *18 years of age *Have no previous type 1 or Type 2 diabetes *Not consuming medications for prediabetes *Have received a blood test result in the prediabetes range in the past year or have been previously diagnosed with gestational diabetes *A fasting blood glucose score of 100 to 125mg/dl *An A1c test resulting in a level of 5.7-6.4%
